• Please read pages 1 and 2 of the application. Complete page 3; you may type directly into the form.
• Once you have completed your portion of the application, print the document
• Turn the tutor application, all portions including the biographical sketch, recommendation, and degree audit into SARC. SARC is located in Howard Phillips Hall, room 113
• Deliver the Letter of Recommendation form to your chosen reference. Instructions for completing this portion are on the form. OR, persons that you choose to serve as references may also download the reference form below, complete it, and deliver it to SARC.
